农村地区艾滋病病毒感染者卫生服务需求、利用及医疗费用的研究

Study on the requirement, utilization and cost of health service of HIV- infected patients in rural areas

目的了解农村地区艾滋病病毒(HIV)感染者卫生服务需求与利用现状,积极探讨适合农村地区HIV感染者卫生服务需求的有效供给模式。方法选择了3个艾滋病综合防治示范区对所有现存的HIV感染者通过问卷调查了解其患病史、就诊史和医疗费用支出等情况。结果两周患病率HIV感染者(78.08%)高于一般人群(34.81%),两周就诊率HIV感染者(69.86%)高于一般人群(31.49%),平均每人年门诊费用HIV感染者(4303.00元)高于一般人群(1224.48元)。结论感染艾滋病促使居民卫生服务需求和利用的上升,但医疗费用的昂贵阻碍了农村地区HIV感染者接受医疗卫生服务,因此尽快出台相关政策提高卫生服务利用率是非常有必要的。

Objective The present study was conducted to investigate the situation of the requirement and utilization of health service for HIV-infected patients in rural areas and actively explore an effective supply mode fit for the requirement of health service for HIV-infected patients in rural areas. Methods Three demonstration regions of comprehensive prevention and control of AIDS were selected, where all survivals with HIV infection were surveyed by questionnaires on their disease histories, therapy histories and medical costs, etc. Results Two-week prevalence of HIV-infected patients （78.08%） was higher than that of general population （34.81%）, two-week visit rate of HIV- infected patients （69.86%） was higher than that of general population （31.49%）, and average annual clinic cost of each HIV-infected patient （4303.00 yuan） was higher than that of general population （1224.48 yuan）. Conclusion The requirement and utilization of medical service of the residents increased because of HIV infection, but high medical cost prevented HIV-infected patients in rural areas from receive medical services. So it is very necessary to establish related policies to improve the utilization rate of health service.
