肝癌的MSCT动态增强与肿瘤血管生成的相关性研究 被引量:1

Correlation between Multislice Helical Computed Tomography Dynamic Enhancement and Tumor Angiogenesis in Hepatocellular Carcinoma

摘要 目的采用多层螺旋CT(MSCT)探讨肝癌动脉期最大强化率与组织病理分级、CD34、血管内皮生长因子(VEGF)表达及微血管密度(MVD)计数的相关性,评价肿瘤血管生成。方法手术病理证实的30例肝癌患者,术前分别应用16层CT的Testblous序列进行同层动态增强扫描,术后肿瘤标本行HE染色及免疫组化分析。结果①肝癌组织的时间密度曲线在动脉期强化的斜率明显高于周边肝组织;②MVD计数、CD34与癌组织动脉期最大强化率呈正相关因素;③肝癌分化差、组织学分级高的患者微血管计数高于分化好、组织学分级低的患者。结论MSCT动态增强扫描可间接判断肿瘤微血管生成的情况,能反映癌组织的生物学特性,且无创、快捷、能重复运用。根据肝癌的MSCT动态增强的征像,可间接评价其相关基因的表达,为其的治疗和预后的判断提供有价值的依据。 Objective To study the relationship between MSCT peak dynamic enhancement ratio in arterial phase and the grade of cell differentiation, MVD, VEGF,CD34 xpressions of HCC, and to evaluate the tumor angiogenesis in hepatocellular carcinoma and provide a foundation to treatment and predicting prognosis of HCC. Methods MSCT dynamic enhancement were performed in 30 patients who were confirmed by histopathology. Immunohistochemical and HE staining were performed in all patients with HCC. Results The peak contrast enhancement ratio of HCC in arterial phase was more obvious. The peak contrast enhancement of HCC in arterial phase was correlated with MVD CD34 expression positively. The higher was the grade of cell differentiation, the more was MVD. Conclusion The tumor angiogenesis and the biological behavior of HCC can be assessed indirectly by MSCT dynamic enhancement. It is a noninvasive ,quick and repeatable way. Genes expression of HCC can be evaluated indirectly, and the valuable foundation to HCC treatment and predicting prognosis can be obtained according to the feature of MSCT dynamic enhancement in HCC.
