糖尿病合并脑卒中的临床特点探讨

Stroke Subtype and its Profiles of Clinical Symptom in Diabetic Patients

目的探讨2型糖尿病(DM)患者合并脑卒中时其卒中亚型及临床表现特点。方法回顾性分析448例初次脑卒中患者的临床资料,按照有无2型糖尿病分成DM组(n=94)与非DM组(n=354)。对两组患者的基本资料、脂质代谢水平、临床表现、卒中亚型进行比较。结果DM组较非DM组更容易发生腔隙性脑梗死(70.9%vs54.3%,P<0.001)和后循环梗死(58.2%vs35.8%,P<0.05),更容易出现肢体偏瘫(46.8%vs41.0%,P<0.01)、构音困难(20.2%vs15.3%,P<0.05)和失语(22.3%vs18.4%,P<0.05)。结论2型糖尿病患者发生脑卒中时,有特征性的临床表现模式,其运动系统功能受损相对较重,影响预后。

Objective To investigate stroke subtype and its profiles of clinical symptom in type 2 diabetic patients. Methods Four hundred and forty eight hospitalized patients for-the-first-time stroke in Shanghai No. 8 People＇s Hospital during 2004-01- 2005-12 were enrolled and analyzed, who were divided into 2 groups with or without type 2 diabetes. Two groups were compared according to fundamental information, risk factors, clinical manifestation and subtypes of stroke. Results More ischemie stroke（70.9% vs 54.3% ,P〈0. 001）, lacunar cerebral infarction （58.2% vs 35. 8% ,P〈0. 001）, limb weakness（46.8% vs 41.0% ,P〈0. 01）, dysarthria（20.2% vs 15.3% ,P〈0. 05） and aphasia（22.3% vs 18.4%, P〈0. 05） occurred in group with diabetes. Conclusion Specific clinical stroke pattern and worse prognosis in terms of damaged motor function are identified in type 2 diabetic patients.
