带状疱疹神经痛的危险因素分析及临床疗效评估

Risk Factor Analysis and Clinical Effect Evaluating of Herpes Zoster Neuralgia

目的回顾性分析带状疱疹神经疼痛的危险因素,评估比较不同方案治疗带状疱疹的临床效果。方法回顾200例带状疱疹患者的临床资料,以多元回归分析法分析年龄、性别、病期等与带状疱疹神经疼痛的相关性,同时统计分析阿昔洛韦联合γ-干扰素治疗和单用阿昔洛韦治疗,比较临床症状改善情况。结果带状疱疹神经痛危险因素的多元线性回归分析显示年龄和是否应用干扰素与疼痛缓解时间存在线性回归关系。阿昔洛韦联合γ-干扰素治疗组患者的疼痛缓解时间和皮疹消退时间明显短于单用阿昔洛韦治疗组(P<0.01)。结论高年龄可能是带状疱疹病人神经痛的主要高危因素之一。阿昔洛韦联合γ-干扰素应用能够显著缩短带状疱疹神经痛和皮疹的持续时间,可成为老年患者和神经痛症状严重患者的推荐方案。

Objective To investigate the risk factors of herpes zoster neuralgia and assess the clinical effects of different treatment methods. Methods Clinical data of 200 herpes zoster patients was analyzed by retrospective study. Multivariate linear regression model was used to analyze the risk factors and clinical effects of mono-therapy of acyclovir and combined acyclovir and T-interferon （IFN-γ） therapy. Results The multiple regression analysis showed that age and IFN-γ use were significantly correlatod with the duration of neuralgia （P 〈 0.05）. The pain-reliof time and skin rash-dlsappearing time of combined therapy group was significantly shorter than that of mono-therapy group （P 〈 0.05）. Conclusions Advanced age may be one of the main risk factors for herpes zoster neuralgia. Combined acyclovir and IFN-γ therapy can shorten the duration of pain and rash of herpes zoster.
