预防经尿道前列腺汽化电切术后大出血的护理因素

Nuring care of Patients of Hemorrhage After Transurethral Vaporization Resection of Prostate

目的 探讨预防经尿道前列腺汽化电切术（TVP）后大出血的护理方法。方法 对180例TVP患者手术前后的护理资料进行回顾性分析。结果本组病例中1例发生术后迟发性大出血，发生率0．56％，经止血、输血等治疗后出血停止，痊愈出院。结论 充分的术前准备，严格的无菌操作及细心配合手术，术后注意保持尿管通畅，避免创面感染，指导督促围手术期应用非那雄胺等护理方法有利于预防TVP术后大出血。

Abstract Objective To investigate the nursing factors in prophylaxising severe postoperative hemorrhage of transurethral vaporization resection of prostate（TVP）. Methods The preoperative and postoperative nursing materials of 180 cases undertaken TVP were analyzed retrospectively. Results Among them, only one case of late severe hemorrhage had occurred, the rate of occurrence was 0.56%. The case was cured by the treatments such as hemostasis and blood transfusion. Conclusions Sufficient preparation , strict aseptic technique and careful nursing coordination for operation were important in prophylaxising the complication. The nursing factors such as keeping the catheter fluently, preventing the infection of the prostate wounds and supervision of finasterid administration during peri-operative period are the key point to prevent and treat the complication of severe postoperative hemorrhage of TVP.
