摘要
[目的]探讨在体内IFN-α2b(干扰素-α2b)对慢性乙肝患者CD25(白细胞分化抗原25)的免疫诱导作用及其抗病毒效果。[方法]采用临床试验将110例典型慢性乙肝患者根据病情分为轻度组(65例),中度组(45例),并给予IFN-α2b治疗3疗程(每疗程12周)。每疗程前后,应用生物素-链霉亲和素(BSA)法和RT-PCR法动态检测体外PHA诱导前后患者PBMC的CD25和CD25 mRNA的表达水平,PCR法动态检测患者外周血单个核细胞(PBMC)内和血清中HBV-DNA。[结果]轻度组和中度组患者经干扰素治疗后的静息态和诱导态CD25表达水平逐渐升高,其CD25 mRNA含量亦同步升高,但与正常对照组仍存在不同差异性(P﹤0.05或P﹤0.01)。IFN-α2b治疗36周后,患者PBMC内、血清中HBV-DNA和HBeAg阴转率两组分别为29(44.62%)、26(40.00%)、39(60.00%)和11(24.44%)、9(20.00%)、17(37.78%),差异有统计学意义(P﹤0.05)。[结论]IFN-α2b可诱导慢性乙肝患者表达CD25,促进T细胞活化发挥抗病毒作用。此效果轻度组高于中度组,似与患者病情程度相关。
[Objective] To explore the effect of IFN-α2b on inducement to CD25 and against HBV of the patients with chronic hepatitis B in vivo. [Methods] The total of 110 patients were divided into two groups (65 patients with slight chronic hepatitis B, 45 patients with moderate chronic hepatitis B), and treated by IFN-α2b for 36 weeks. Before and after PBMC being stimulated with PHA in vitro, the expression of CD25 and its mRNA of PBMC was detected by method of biotin- streptavidin (BSA) and RT-PCR. The levels of HBV-DNA in PBMC and serum were dynamically detected by PCR during the treatment. [Results] The expression of CD25 and its mRNA gradually increased after treatment of IFN-α2b. However, there was significant difference between two groups and the normal control group (P 〈 0.05 or 0.01 ) in silence or in inducement. After the treatment of IFN-α2b for 36 weeks, the total negative rates of HBV-DNA in PBMC, serum and HBeAg of the patients in two groups were 29 (44.62%), 26 (40.00%), 39 (60.00%), and 11 (24.44%), 9 (20.00%), 17 (37.78%) respectively. There was significant difference between two groups (P 〈 0.05) . [Conclusion] There may be a link between the level of CD25 and the pathogenetic condition of patients with chronic hepatitis B. CD25 of active T cells of the patients with chronic hepatitis B can be induced by IFN-α2b, and it plays key role against HBV-DNA in the host's body. Meanwhile, patients with slight chronic hepatitis B have better therapeutic effect.
