摘要 [目的]评价实时荧光定量PCR检测方法在诊断女性淋球菌(NGH)感染的效果和可行性,试图寻找一种快速而敏感的淋球菌检测方法。[方法]用革兰染色涂片法、培养法和FQ-PCR法检测女性疑诊淋病患者,其中FQ-PCR法采用宫颈拭子、阴道拭子和尿液3种取材方法进行对比。以培养法为“金标准”,分别计算其敏感性、特异性、阳性预期值和阴性预期值。并对部分患者治疗前后定量结果进行分析。[结果]培养法检测淋球菌的检出率为6.7%,涂片法的敏感性为60.00%,阳性预期值为54.55%。宫颈拭子FQ-PCR、阴道拭子FQ-PCR和尿液FQ-PCR法的敏感性分别为90%、95%和80%,3种FQ-PCR法特异性高达98%。[结论]FQ-PCR法检测女性淋球菌,具有快速、定量准确、较高敏感性和特异性等优点,可弥补涂片法较粗糙、培养法耗时长的缺点。是一种具有临床应用价值的淋球菌快速定量检测方法。 [Objective] To evaluate the clinical practical value of real-time fluorescent quantitative polymerase chain reation (FQ-PCR) in detection of woman patients gonococcal infection, and to find a fast and sensitive method to detect gonococcus. [ Methods] Urogenital specimens were obtained from patients with suspected gonorrhoeae .These specimens were tested by smear method, cultivation and FQ-PCR. Cervical swabs, vaginal swabs and urine-based were detected with FQ-PCR. Cultivation was considered as the "gold standard" . Compared with the cultivation, the sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were calculated. [ Results] The positive rate of cultivation was 6.7%.In comparison with the cultivation, the sensitivity and PPV were 60.00% and 54.55% for smear method. The sensitivity were 90%, 95% and 80% for cervical swabs FQ-PCR, vaginal swabs FQ-PCR and urine-based FQ-PCR. The specificity was more than 98% for the three FQ-PCR detections. [Conclusion] The FQ-PCR detection possess features of accuracy, fast, higher sensitivity and higher specificity. The FQ-PCR detection was more precise than the smear detection method and costs less time than the cultivation. FQ-PCR provides a fast method which is feasible in clinical practice for detection of gonococcus.
