妊娠期糖耐量受损患者孕中期体重变化的分析

Clinical observation on the relationship between weight gain during pregnancy with gestational impaired glucose tolerance

目的 了解妊娠期糖耐量受损患者孕中期体重变化的规律。方法 收集2001年1月至2005年6月住院分娩病例共210例的临床资料，其中妊娠期糖尿病（GDM）70例，糖耐量低减70例，正常妊娠70例，回顾对照分析这些患者在妊娠16～28周体重变化的差异。结果将孕期分为16周以前、16—20周、20～24周、24～28周4个阶段，计算每个阶段中增加的体重。对比分析妊娠期糖尿病组与正常孕妇组，糖耐量低减组与正常孕妇组，妊娠期糖尿病组与糖耐量低减组。发现妊娠期糖尿病组与糖耐量低减组在以上两个阶段中每周增加的体重均明显高于正常孕妇组（P〈0．05），而妊娠期糖尿病组又高于糖耐量低减组（P〈0．05）。结论 自妊娠16周开始如果体重增加愈明显，患妊娠期糖尿病与糖耐量低减的可能性愈大。应高度重视妊娠20周前的体重增加幅度。

Objective To study the influence of gestation diabetes mellitus （ GDM ） and gestation impaired glucose tolerance to weight gain of pregnant women . Methods All non-diabetic pregnant women attending antenatal clinics from 2001 to 2005 （n =210） were screened for GDM during the 24th to 28th weeks of gestation. All positive subjeers based on a 50g glucose challenge test （GCT） were further evaluated by a diagnostic 75g oral glucose tolerance test （ OGTT）. Results Women who developed GDM had significantly higher BMI in the pre-pregnancy stage and in the second and third trimester of pregnancy （ P 〈 0.001 ）. Conclusion GDM and obesity are highly prevalent in pregnant women.
