溃疡性结肠炎患者基质金属蛋白酶-1表达测定的意义 被引量:1

摘要 目的检测基质金属蛋白酶-1(MMP-1)在溃疡性结肠炎(UC)患者结肠组织中的表达,探讨MMP-1在溃疡形成过程中的作用以及与病情严重程度之间的关系。方法采用逆转录-聚合酶链反应(RT—PCR)和免疫组织化学染色法在40例UC患者的结肠黏膜溃疡区、炎症区以及相对正常区取活组织在mRNA水平及蛋白水平上测定MMP-1表达,以10例正常人的结肠组织(正常对照组)作为对照,分析MMP-1的表达与病情严重程度之间的关系。结果在mRNA水平上,MMP-1在溃疡区、炎症区结肠组织中的表达较相对正常区明显增加(P〈0.01),较正常对照组也明显增加(P〈0.01);在蛋白水平上,MMP-1在溃疡区、炎症区的表达较相对正常区明显增加(P〈0.05),较正常对照组也明显增加(P〈0.05);相对正常区与正常对照组之间的表达无论在mRNA水平还是在蛋白水平差异均无统计学意义;MMP-1的表达在溃疡区、炎症区、相对正常区及正常对照组结肠组织中的阳性率分别为85%、82%、58%、40%;MMP-1与UC病情严重程度具有相关性(r=0.406,P〈0.05)。结论UC病变结肠组织中MMP-1的表达增加,并且与UC的组织损伤程度有关,MMP-1可以作为评价UC病情严重程度的生物学指标;应用外源性基质金属蛋白酶抑制剂治疗可能会成为UC治疗的新途径。 Objective To examine the expression of matrix metalloproteinase - 1 ( MMP - 1 ) in colonic mueosa of patients with ulcerative colitis(UC) , to investigate the role of MMP - 1 in the process of ulceration, and to verify the relationship between MMP- 1 expression and the severity of the disease. Methods Reverse transcription - polymerase chain reaction ( RT - PCR) and immunohistoehemistry were used to study, at both mRNA and protein level, the expression of MMP - 1 in the ulcerated, inflamed and non - inflamed area of colonic mucosa in 40 patients with UC. Biopsy specimens from 10 normal subjects were selected as normal control group. The relationship between MMP - 1 the severity of the disease was analyzed by statistical methods. Results Expression of MMP - 1 mRNA in the ulcerated and 'inflamed colonic mucosa was significantly higher than that in non - inflamed colonic mucosa ( P 〈 0.01 ) and normal control group(P 〈 0.01 ). Expression of MMP - 1 at the protein level in the ulcerated and inflamed colonic mucosa was significantly increased when compared with that in non - inflamed colonic mucosa (P 〈 0.05)and in the normal control group(P 〈 0.05 ). Neither at the mRNA level nor at the protein level the expression of MMP - 1 was statistically different in non - inflamed colonic mucosa and in the normal controls. In the ulcerated, inflamed, non - inflamed colonic mucosa and in the control, the positive rates of MMP - 1 expression were 85% , 82%, 58% , and 40% respectively. Furthermore, the expression of MMP - 1 was well related to the severity of clinical symptoms ( r = 0. 406,P 〈 0.05 ). Conclusions The increased expression of MMP - 1 is related to colonic tissue damage in UC. MMP - 1 could be used as a biomarker to judge the severity of the disease. Treatment with matrix metalloproteinase inhibitors could be a new way in treating UC.
