微创穿刺术加尿激酶血肿清除与小骨窗开颅术治疗高血压脑出血的对照研究

Comparative study of micro-traumatic hematoma cleaning operation and craniotomy with small bone flap approach microsurgical treatment for hypertensive intracerebral hemorrhage

【目的】探讨微创穿刺术加尿激酶血肿清除与小骨窗开颅显微镜下血肿清除术在高血压性脑出血治疗中的疗效对比。【方法】2000年1月-2004年12月84例符合手术适应征的高血压性脑出血急性期患者被随机分为微创术加尿激酶血肿清除治疗组(穿刺组)和小骨窗开颅显微镜下血肿清除(小骨窗组)手术组,比较两组治疗后的临床转归。【结果】微创术加尿激酶血肿清除治疗组疗效与小骨窗开颅术清除血肿疗效差异无统计学意义(P>0.05)【结论】微创术加尿激酶血肿清除治疗高血压脑出血与小骨窗开颅显微镜下血肿清除疗效相近。

[Objective] To evaluate the effectiveness and safety of micro-traumatic hematoma cleaning operation and craniotomy with small bone flap approach microsurgical treatment for hypertensive intracerebral hemorrhage. [Methods] In a prospective clinical trail, 84 patients with hypertensive intracerebral hemorrhage were randomly assigned into two groups to receive small bone flap approach microsurgical treatment or micro-traumatic hematoma cleaning operation from Feb. of 2000. 1 to July of 2004. 12. [Results] There is no statistically significant （ P 〉 0.05） between the two arms. [Conclusions] The micro- traumatic hematomas cleaning operation can obtain similar therapeutic effect as compared with small bone flap approach microsurgical treatment for hypertensive intracerebral hemorrhage.
