哈萨克族高血压病患者血浆脂联素水平及其相关性研究

摘要 目的分析新疆哈萨克族(哈族)高血压病(EH)患者血浆脂联素(APN)水平及其与胰岛素抵抗(IR)、血糖、血脂、血压、肥胖的关系。方法以新疆哈族EH患者67例为观察组,哈族健康人群60例为对照组。测量血压、身高、体质量、腰围(WC)及臀围。抽取空腹静脉血,测定血浆APN、胰岛素(FINS)、血糖(FPG)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A1(Apo-A1)和载脂蛋白B(Apo-B),计算体质量指数(BMI)、腰臀比(WHR)、体内脂肪百分比(BF%)和HOMA-胰岛素抵抗指数(HOMA-IR)。结果①EH组血浆APN明显低于对照组(P<0.01);②EH组血浆APN与收缩压(SBP)、舒张压(DBP)、WC、BMI、WHR、FPG、FINS、HOMA-IR、TG、LDL-C、Apo-B呈显著负相关;对照组血浆APN与SBP、DBP、WC、BMI、WHR、FINS、HOMA-IR呈显著负相关;③多元回归分析显示:在EH组中,HOMA-IR和WC是影响血浆APN水平的独立因素;在对照组中,FINS和BMI是影响血浆APN水平的独立因素。结论哈族EH患者血浆APN水平降低并与血压之间存在一定的相关性。 Objective To explore the plasma adiponectin(APN)levels in the patients with essential hypertension (EH) of Xinjiang Kazakans and the relations between plasma adiponectin level and insulin resistance(IR), blood glucose,blood lipids, blood pressure and obesity. Methods Totally 127 Kazakans EH cases were selected for the subjects. They were divided into two groups according to their blood pressure: EH group ( n =67) and control group ( n = 60). Measurment of blood pressure, body height, body mass, waist circumference (WC) and hip. Fasting venous blood was drawn to detect the plasma APN, fasting insulin(FINS), fasting plasma glucose (FPG), total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol ( HDL-C), low-density lipoprotein cholesterol ( LDL-C), apoprotein-A1 (Apo-A1) and apoprotein-B( Apo-B), calculating body mass index (BMI), waist-hip ratio (WHR), body fat percentage (BF%) and homeostasis model assessment of insulin resistance index(HOMA-IR). Results ① The plasma adiponectin concentration in E H group was significantly lower than that in control group( P 〈 0. 01 ). ②APN was negatively and significantly correlated with systolic blood pressure(SBP), diastolic pressure(DBP), WC, BMI, W HR, FPG, FINS, HOMA-IR, TG, LDL-C, Apo-B in EH group, and negatively and significantly correlated with SBP, DBP, WC, BMI,WHR, FINS, HOMA-IR in control group. ③ Multiple regression analysis showed that HOMA-IR and WC were independent factors to affect the APN level in EH group, FINS and BMI were independent factors to affect the plasma adiponectin level in control group. Conclusions Concentration is decreased in the patients with EH of Kazakans ,and there is correlation between APN and blood pressure.
