扩大翼点入路治疗额颞部重型颅脑损伤分析

Analysis of craniectomies via extended pterional approach in the patients with severe brain injury of fronto-temporal side

目的探讨扩大翼点入路开颅术对额颞部重型颅脑损伤患者的治疗效果。方法对86例额颞部重型颅脑损伤患者行扩大翼点入路开颅术,清除血肿和去骨瓣减压。结果86例患者中,恢复良好44例(51.2%),重残20例(23.3%),植物生存8例(9.3%),死亡14例(16.3%)。结论扩大翼点入路手术治疗额颞部重型颅脑损伤具有暴露广泛、减压充分、术中脑膨出发生率低、疝容易复位等优点,从而降低患者病死率及病残率,提高手术治疗效果,提高患者生存质量,降低死亡率。

Objective To investigate the effect of craniectomies via extended pterional approach on the patients with severe brain injury of fronto - temporal side. Methods 86 patients with severe brain injury of fronto - temporal side received craniectomies via extended pterional approach to remove the hematoma and osseous flap. Results There were 44 cases（ 51.2% ） with good recovery （ including moderate disability ）, 20 cases （ 23.3% ） with severe disability, 8 cases （ 9.3 % ） with vegetative state, and 14 cases （ 16.3% ） died. Conchlsiolm The method of craniectomies via extended pterional approach has many advantages in the treatment of patiems with severe brain injury of fronto - temporal side. It could lead to the exposure of the injury side sufficiently, reduce the incidence of cephalocele and be helpful to the repo- sition of cerebral hernia, improve the operation effect, increase the life quality and decrease the mortality.
