经腹腹腔镜肾癌根治术(附64例报告)

Transperitoneal laparoscopic radical nephrectomy (Report of 64 cases)

目的：探讨经腹腹腔镜肾癌根治术的技术要点及临床价值。方法：肾癌64例，肿瘤平均直径5．6cm；左侧29例，右侧35例，其中58例肿瘤直径小于7．0cm，6例肿瘤直径大于7．0cm，均采用经腹腹腔镜肾癌根治术进行治疗。结果：64例全部成功，未中转开放手术。手术时阃90-230min．平均130min。术中及术后均未输血，无严重并发症。术后住院4～8天，平均6．2天。结论：经腹腹腔镜肾癌根治术具有损伤小、安全有效、可行、恢复快等优点，具有良好的临床应用前景。

Objective：To evaluate the surgical technique and clinical application of transperitoneal laparoscopic radical nephrectomy. Methods：Sixty-four cases of renal carcinoma were included in this group, 29 cases having left kidney lesion and 35 having right with a mean diameter of 5.6 cm. Fifty eight cases had tumors size less than 7cm in diameter and 6 cases more than 7cm. All 64 patients underwent transperitoneal laparoscopic radical nephrectomy. Results ： All the operations were successful in 64 cases of patients. The operation time was 90 - 230 minutes with a mean of 130 minutes. All patients did not need blood transfusion and no severe complication was observed. The median postoperative hospital stay was 4 -8 days with average length of 6.2 days. Conclusions：Transperitoneal laparoscopic radical nephreetomy has advantages of minimal invasive, safe and effective, postoperative early resumption and shorter hospital stay, which has a promising future in the urologic clinical practice.
