地塞米松对脓毒症大鼠急性肾功能衰竭细胞因子的影响 被引量:2

摘要 目的研究脓毒症大鼠急性肾功能衰竭时炎症介质和抗炎细胞因子之间的关系以及地塞米松(DXM)对细胞因子表达的调控作用。方法雄性SD大鼠72只随机分成对照组(A组)、LPS模型组(B组)、DXM+LPS治疗组(c组),各组在2h、4h和6h时观察8只大鼠肾组织大体外观及病理学变化,测定血肌酐及血钾浓度的变化,同时测血清细胞因子TNF-α、IL-6、IL-10的浓度。结果B组肉眼观肾脏表面色泽暗红,可见包膜下点状、片状出血;C组肾脏表面有轻度充血、水肿;和A组比较,B组肾脏病理呈急性炎症改变,C组肾脏病变较B组明显减轻。在4、6h时点,B组和C组血肌酐及血钾浓度与A组比较明显上升(P〈0.05),但在4、6h时点B组血肌酐及血钾浓度高于C组(P〈0.05)。B组血清中IL-6、IL-10和,TNF-α较A组明显升高(P〈0.05),C组较B组IL-6和TNF-α浓度显著降低(P〈0.05),IL-10浓度明显增高(P〈0.05)。结论DXM可以通过下调炎症因子的表达,上调抗炎因子的表达,抑制中性粒细胞在肾脏的浸润而保护脓毒症大鼠的肾功能。 Objective To investigate the relationship between inflammatory cytokines and anti-inflammatory cytokines in septic rats with renal failure, and the effects of dexamethasone on cytokines. Method Seventy-two male SD rats were randomly divided into three groups: group A (normal group, n =24), group B (LPS group, n = 24) and group C (LPS + DXM, n = 24). The pathological changes of kidney tissue, blood creatinine and potassium concentration, blood concentration of TNF-α, IL-6, IL-10 were observed at 2 hours, 4 hours and 6 hours after LPS given. Results The pathological changes of kidney tissue in group C were obviously slighter than those in group at B 4 hours and 6 hours after LPS given. Blood creatinine and potassium concentration in group B and group C were higher than those in group A at 4 and 6 hours after LPS given, while group C was lower than group at B 4 and 6 hours after LPS given. Blood concentration of TNF-α and IL-6 in group C declined more obviously than those in group B at 4 and 6 hours after LPS given, while blood concentration of IL-10 increased significantly in group C. Conclusions DXM could protect the renal function in LPS-induced septic rats with acute renal failure, which was likely due to the downregulated expression of cytokines and upregulated expression of antiinflammatory cytokines.
