CT诊断真菌性鼻窦炎

CT Findings of Fungal Sinusitis

目的探讨真菌性鼻窦炎CT影像表现特征,提高影像诊断水平。方法回顾分析真菌性鼻窦炎22例,手术病理证实13例,组织活检9例。全部病例均行全组鼻窦CT扫描及病灶处高分辨CT(HRCT)检查。结果上颌窦20例(90.91%),蝶窦2例(9.09%),筛窦2例(9.09%),额窦1例(4.55%)。病变多为单侧(68.18%)。鼻窦内软组织密度肿块22例,网格状高密度影3例,病灶内点状或斑块状钙化灶16例,窦壁骨质增生硬化9例,窦壁骨质破坏6例,侵犯邻近眼1例。结论CT可对真菌性鼻窦炎做出定性定位诊断,HRCT对此病诊断与常规CT相比不具明显优势。

Objective To analyse and evaluate the CT findings in diagnosis of fungal sinusitis and to improve its radiological diagnosis. Methods The CT findings of 22 cases of fungal sinusitis were retrospectively analyzed. The diagnosis in all cases was confirmed by surgery （ 13 cases） or biopsy （9 cases）. All of the cases had CT scan and HRCT additionally. Results Antrum highmori 20 cases（90. 91% ）, sphenoid sinus 2 cases （9.09%） ,antrum ethmoidale 2 cases（9.09% ） ,frontal sinus 1 case（4.55% ）. Most lesions are half（68.18% ）. The CT findings mainly include soft tissue in the paranasal sinus （22 cases）, web - like hyperdense （ 3 cases ）, calcification （ 16 cases ） , osteohyperplasia （9 cases）. Bone destruction （6 cases）. Eye invasion （ 1 case）. Conclusion Fungal sinusitis can be correctly diagnosed by CT scan, but HRCT is not superior over CT scan in the final diagnosis and determination of the extent of lesion.
