外伤性脑积水证治初探

Treatment of Traumatic Hydroceplalus Based on the Traditional Chinese Medicine Differential Diagnosis

采用回顾性研究方法,统计356例脑挫裂伤合并蛛网膜下腔出血患者,结果出现急性脑积水者6例,慢性脑积水者35例。分析认为昏迷患者给以流质饮食、小便不利、高热是颅脑外伤后发生脑积水的主要因素,并从瘀阻清窍、脾虚水聚、肾阳不足膀胱失司、热毒壅滞四个方面对其病因病机与辨证论治进行了分型探讨。

The data of 356 patients with cerebral contusion complicating subarachnoid hemorrhage were analyzed retrospectively. Among them ,6 cases acute hydrocephalus and 35 cases chronic hydrocephalus were summarized. After the data analysis we can conclude that the liquid diet given to the patients in coma, unsmooth urination and high fever were main fators of traumatic hydrocephalus. According to TCM etiology and pathogenesis, stasis stagnating brain, spleen deficiency inducing retention of water,insufficiency of kidney yang complicating urinary bladder incontinence,stagnation and coagulation of heat and toxin were four clinical types ,treatments of each type based on TCM differential diagnosis were studied in this article.
