两种三联疗法治疗幽门螺杆菌相关性消化性溃疡临床观察

The Clinical observation of Two Trilogy Therapy Against Helicobacter pylori Relative Peptic Ulcer

目的比较两种三联疗法治疗幽门螺杆菌相关性消化性溃疡的疗效。方法将幽门螺杆菌阳性的消化性溃疡病例120例分成两组,A组用奥美拉唑+克拉霉素+阿莫西林口服治疗,B组用奥美拉唑+呋喃唑酮+阿莫西林口服治疗,疗程均为7天。观察消化性溃疡主要症状缓解情况、Hp根除率、溃疡愈合率、有效率及药物不良反应。结果A、B两组消化性溃疡主要症状上腹痛、嗳气、反酸、恶心、腹胀的缓解率分别为91.5%和89.3%,90.0%和94.7%,85.7%和79.3%,90.9%和90.7%,85.7%和92.5%,差异无统计学意义(P>0.05);Hp根除率分别为91.7%和88.3%,差异无统计学意义(P>0.05);溃疡治愈率分别为66.7%和60.0%,总有效率分别为96.7%和93.3%,差异无统计学意义(P>0.05);不良反应发生率分别为10.0%和8.3%,差异无统计学意义(P>0.05)。结论两组方案均能有效缓解消化性溃疡症状和根除Hp,B组中因为呋喃唑酮价格低廉,不良反应轻,可推荐为治疗Hp相关性消化性溃疡的一线用药。

Objective To compare the therapeutical effect of two trilogy therapy against Helicobacter pylori relative peptic ulcer. Methods We assigned 120 cases of peptic ulcer patients whose tests of Helicobacter pylori showed positive into two groups. Patients in group A was administrated with omeprazole, Clarithromycin and amoxicillin for seven days by oral. Patients in group B was administrated with omeprazole, furaltadone and amoxicillin for seven days by oral. We determined to observe the primary symptomatic relief of peptic ulcer, eradication rate of Hp, recovery rate of ulcer, efficiency and drug adverse reaction. Results The primary symptoms of peptic ulcer in both group A and B include upper abdominal pain, belching, sour regurgitation, nausea, abdominal distention. The relief rate of these symptoms in group A and B were 91. 5 % and 89.3 %, 90.0 % and 94.7 %, 85.7 % and 79.3 %,90.9 % and 90.7 %, 85.7 % and 92.5 % ,respectively. Eradication rates of Hp were 96.7% and 88.3% , recovery rate of ulcer were 66.7% and 60.0% , total efficiency were 96.7% and 93.3% , incidence of drug adverse reaction were 10.0% and 8.3% , respectively, significant difference was not observed between the two groups （ P 〉 0.05 ）. Conclusions Both A and B therapy schedules were able to effectively relieve symptoms of peptic ulcer and eradicate the level of Hp. Since the inexpensive price and gently adverse reaction of frahadone in group B, it could be recommended to be the first - choice drug in curing Hp relative peptic ulcer.
