重组人生长激素治疗前后矮身材儿童血清钙及锌的变化

摘要 目的探讨生长激素缺乏症(Growth hormone deficiency,GHD)患者、非生长激素缺乏性矮身材(Non-GH deficient short stature,NGHDSS)患者及GHD患者应用生长激素治疗前后的钙、锌变化。方法20例GHD患者在治疗前及治疗3月后分别测钙、锌水平并观察他们的身高变化,并与20例NGHDSS患者和20例正常儿童进行对照。钙、锌采用生化仪测定。结果GHD患者治疗前血清钙水平与正常对照组无显著性差异(P=0.54),治疗3个月后血清钙水平较治疗前稍减低,但元显著性差异(P=0.892)。NGHDSS患者血清钙水平与GHD组无显著性差异(P=0.892),与正常对照组无显著性差异(P=0.64)。GHD患者治疗前血清锌水平与正常对照组无显著性差异(P=0.42),治疗3个月后血清锌水平较治疗前增加,有显著性差异(P=0.042,P<0.05)。NGHDSS患者血清锌水平与GHD组无显著性差异(P=0.75),与正常对照组无显著性差异(P=0.48)。结论①GHD及NGHDSS患者血清钙水平正常,GHD患者短期治疗后略有降低,提示GHD患者治疗早期应适当补充钙制剂;②GHD及NGHDSS患者血清锌水平正常,GHD患者治疗后血锌水平上升,提示锌缺乏不是引起儿童严重矮身材的主要原因,生长激素治疗可促进锌的吸收。 Objective To study the changes of the serum calcium (Ca), zinc (Zn)in growth hormone deficiency(GHD), non-GH deficient short and slight in figure(NGHDSS) and GHD children after 3 months of r-hGH replacement treatment.Methods The serum levels of Ca,Zn were measured in 20 GHD children before and after 3 months of r-hGH therapy, and compared with that of 20 cases of NGHDSS and 20 normal children. Ca,Zn were measured by biochemistry equipment. All calculation were performed with SPSS11.0.Results There was no difference of the serum levels of Ca in GHD group with those in NGHDSS and control group and r-hGH treatment group.The serum level of Zn increased significantly in GHD group after 3 months of r-hGH replacement therapy (P=0.042,P〈0.05).Conelusion (l)The serum levels of Ca are normal in children with GHD and NGHDSS,and decrease slightly after 3 months of r- hGH replacement therapy. Ca should be added to GHD children in the early period of therapy.(2) The serum level of Zn is normal in children with GHD and NGHDSS, and increase significantly after 3 months of r-hGH replacement therapy.Lack of Zn is not the main cause of severe short and slight in figure in children;GH can promote the absorption of Zn.
