Ⅲ型前列腺炎辅助性T细胞亚群的分化

The differentiation of helper T lymphocyte subgroup of type Ⅲ prostatitis

摘要 目的了解Ⅲ型前列腺炎前列腺液中CD4+T辅助性T细胞(Th细胞)亚群的分化情况。方法按照美国国立卫生院(NIH)分类方法将病例分成ⅢA型/组(47例)、ⅢB型/组(29例),另设健康对照组(16例)。采用双抗体夹心酶联免疫法检测前列腺液(EPS)中Th1类细胞因子(IFN-γ)、Th2类细胞因子(IL-4)水平以及Th1/Th2比值(IFN-γ/IL-4)。结果和对照组比较,ⅢA组、ⅢB组IFN-γ水平明显上调(P<0.05),ⅢA组较ⅢB组升高更明显(P<0.05);和对照组比较,ⅢA组IL-4水平无明显变化(P>0.05),ⅢB组IL-4水平上调(P<0.05);和对照组比较,ⅢA组IFN-γ/IL-4明显升高(P<0.05),ⅢB组IFN-γ/IL-4无明显改变(P>0.05)。结论ⅢA型前列腺炎Th1细胞分化占优势,Th1/Th2平衡向Th1漂移,以细胞免疫反应为主;Th细胞分化也参与了ⅢB型前列腺炎的发病,但Th1/Th2处于相对平衡状态。 Objective To evaluate the differentiation of CD4^+T helper cell in expressed prostatic secretions(EPS) of type Ⅲ prostatitis. Methods Cases from clinic of the 2nd affiliated hospital of wenzhou medical college were studied during the time of April to Oct. 2005. Cases were classified as type/group Ⅲ A(47 cases) and type/group Ⅲ B(29 cases), they were compared with control group (16 cases ) according to NIH classification system. Thl cytokine IFN-γ ; Th2 cytokines IL-4 in EPS were detected by double antibody sandwich ELISA and IFN-γ /IL-4 was determined. All Th cytokines and IFN-γ/IL-4 were compared in groups. Results Compared with control group, IFN-γ in group Ⅲ A and Ⅲ B were significantly upper adjusted (P〈0.05), wile IFN-γ level in group ⅢA was higher than in group Ⅲ B(P〈0.05); Control group IL-4 in group ⅢA was not obviously alterable(P〉0.05), IL-4 in group Ⅲ B was significantly upper adjusted (P〈0.05); IFN-γ / IL-4 in group Ⅲ A was elevated significantly (P〈0.05), wile IFN-γ / IL-4 in group Ⅲ B was not bviously alterable(P〉0.05). Conclusion Th1 cell differentiation take the dominance in type ⅢA prostatitis ,Th1/Th2 equilibrium is shifted to Th1, cytoimmunity is the main reaction. Th cell differentiation also take part in morbidity of type Ⅲ B prostatitis ,while Th1/Th2 stays in balanced condition.
