摘要
目的:探讨肺动脉栓塞(PE)的螺旋CT血管造影(CTA)的诊断价值。方法:对22例临床疑为PE的患者行螺旋CT血管造影,其中8例在溶栓治疗后进行复查。结果:22例患者累及62处肺动脉及其分支。PE的CT直接征象为:血管腔内充盈缺损,包括部分充盈缺损、附壁充盈缺损、完全阻塞及中心性充盈缺损(轨道征)。其中轨道征提示急性PE,其余3种提示亚急性、慢性PE的征象,对指导临床治疗有重要意义。PE的间接征象为:肺动脉高压、肺梗塞灶及右心室增大。结论:螺旋CT血管造影是一种快速、有效、无创伤性检查肺栓塞的方法,对指导临床正确诊断及治疗后随访均有重要意义。
Objective： To discuss the clinical value of spiral CT angiography in the diagnosis of pulmonary embolism（PE）. Methods： 22 patients of clinically suspected PE underwent spiral CT angiography. 8 the patients were reexamined after thrombolysis. Results： 62 pulmonary arteries and their vessels in 22 cases were involved in PE. Direct signs of PE showed intraluminal filling defect, including partial filling defect, mural filling defect, total occlusion and railway - track sign. The railway - track sign indicated acute PE, and the other three signs indicated sub - acute and chronic PE. It was very important to distinguish chronic PE with acute PE for directing clinical treatment. Secondary signs of PE included pulmonary hepertension, focus of pulmonary infarction and enlargement of right ventricle. Conclusion： Spiral CT angiography is a fast, effective, and non- invasive diagnostic method for PE, It is valuable in for clinical diagnosis and assessment of prognosis.
