关节镜下重建前交叉韧带53例

Reconstruct anterior cruciate ligament by arthroscopy in 53 cases

目的探讨关节镜下重建前交叉韧带（ACL）的手术方法及疗效。方法关节镜下保留ACL残端作为定位标志物。以自体四股胴绳肌腱为ACL重建替代物，应用可吸收界面螺钉固定，对53例ACL损伤病例行重建术。结果随访6。32个月，平均13．5个月。术后膝关节活动度均在正常范围，无韧带撞击现象。Lachman试验：46例≤＋，7例＋＋。轴移试验术后全部阴性。Lysholm评分由术前的平均50．6分提高到术后的平均90．8分，差异有统计学意义（P〈0．05）。结论关节镜下保留少许ACL残端作为重建前交叉韧带定位标志物，定住准确，方法简单，是提高前交叉韧带重建术成功率较好的方法。

Objective To explore the surgical techniques and effects of treatment with reconstruction of ACL. Methods Reserving deformity of ACL for the locating sign and a technique of arthroscopic reconstruction of ACL using the four - stranded hamstring tendond was used in 53 cases. The tendonds were fixed by absorbable screw. Results Followed -up for 6 -32 months（ mean 13.5 months） in all 53 patients showed normal motion range of knee joint without ligament hitting and pivot shift tests were all normal, lacman test revenled ≤ ＋ in 46 cases and ＋ ＋ in 7 cases. The Lysholm score improved from 50.6 to 90.8 before and after operation and the distinction had statistical significance （ P 〈 0.05 ）. Conclusion It is a better technique to improve the rate of success of reconstruct ACL by way of reserving little deformity of ACL for the locating sign by arthroscopy which signing accuracy and operating simplicity.
