凉血解毒汤治疗急性再生障碍性贫血的机制探讨 被引量:14

摘要 目的探讨凉血解毒汤治疗急性再生障碍性贫血的机制。方法采用酶联免疫法检测急性再生障碍性贫血患者及正常对照者骨髓上清液中肿瘤坏死因子α(TNF-α)、γ干扰素(IFN-γ)、可溶性白细胞介素2受体(sIL-2R)的含量,并观察凉血解毒汤在体外对骨髓单个核细胞经植物血球凝集素诱生的造血负调控因子的调变作用。结果急性再生障碍性贫血患者骨髓上清液中TNF-α、IFN-γ、sIL-2R的含量明显高于正常对照者,差异有显著性意义(P<0.05);体外试验显示,在多数急性再生障碍性贫血患者中凉血解毒汤可明显抑制IFN-γ、sIL-2R的分泌(P<0.05或P<0.01),而对TNF-α无明显影响(P>0.05)。结论急性再生障碍性贫血的发病机制与造血负调控因子(TNF-α、IFN-γ、sIL-2R)的异常增多有关;凉血解毒汤在凉血止血、清热解毒的同时,也可能通过下调IFN-γ、sIL-2R的含量而促进造血功能恢复,起到治疗急性再生障碍性贫血的作用。 Objective To study the mechanism of Liangxue Jiedu Decoction for acute aplastic anemia. MethOds Enzymelinked immunosorbent assay was used to detect TNF-α,IFN-γand sIL-2R in myeloid supernatant of acute aplastic anemia patients and normal controllers. Modulation of Liangxue Jiedu Decoction on myelomonocytic TNF-α,IFN-γand sIL-2R induced by plant hemagglutinin was observed in vitro test. Results TNF-α,IFN-γand sIL-2R in myeloid supernatant of acute aplastic anemia patients were higher than those of normal controllers with significant differences(P〈0.05). Vitro test showed that Liangxue Jiedu Decoction inhibitted the secretion of IFN-γ and sIL-2R significantly(P〈0.05 or P〈0.01) ,and had no effect on INF-α(P〉0.05). Conclusion The pathogenesis of acute aplastic anemia is relevant to abnormal increasing of TNF-α,IFN-γ and sIL-2R. Liangxue Jiedu Decoction can cool the blood, stanch bleeding,clear heat and resolve toxin to cure acute aplastic anemia,which may be concerned with its decreasing IFN-γ and sIL- 2R and improving haematopoiesis as well
