肺栓塞的诊断与治疗

Diagnosis and therapy on pulmonary embolism

目的探讨肺栓塞(PE)的易患因素和诊断、治疗中的若干问题。方法回顾总结1995年3月～2005年10月期间23例PE诊治过程与原发和伴发病症相关情况的临床经验。结果本组23例PE患者中伴有下肢静脉血栓和心房纤颤的发生率分别是30.4%和21.7%。螺旋CT诊断有8例被选择性肺动脉造影证实;13例PE静脉溶栓治疗,存活9例,死亡4例;手术治疗8例死亡3例。结论下肢静脉血栓和心房纤颤为PE的高易患因素;螺旋CT对PE有较高诊断价值;外科治疗PE是行之有效的方法。

Objective To discuss susceptible factors to pulmonary embolism （PE） and its diagnoses and therapies. Methods To and summarize the clinical experience in 23 cases of PE therapeutic procedure and related situation of the primary and concomitant diseases,during the period from March 1995 to October 2005. Results The incidence,in this group accompanied with lower limb venous thrombus or atrial fibrillation, is 30.4 % and 21.7% respectively. There are 8 cases diagnosised with Spiral CT confirmed by selective pulmonary arteriography;9 cures and 4 deaths in 13 cases of PE venous thrombolytic therapy; 3 deaths in 8 cases of surgery. Conclusion The lower limb venous thrombus and atrial fibrillation are the most susceptible factors to PE;Spiral CT is of great diagnostic value for PE and surgical operation is the effective method for the PE therapy.
