Case Study of Maintaining SOAP Pharmaceutical Care Records

目的:探讨SOAP式药历的书写思路及方法。方法:结合感染科具体病例进行分析。结果与结论:临床病例中出现的多种问题都可以采用SOAP式药历进行书写,其为一种较好的药历书写方法,有利于体现药学服务理念,保证患者用药安全、有效。

OBJECTIVE： To discuss the how＇ s on writing and maintaining SOAP- type pharmaceutical care records in clinical practice. METHODS： An analysis was made on how to write and maintain SOAP- type pharmaceutical care records through case studies of the practice adopted by the infection department of our hospital. RESULT ＆ CONCLUSIONS： SOAP is a proven protocol suitable for writing and maintaining pharmaceutical care records, which can not only record various problems occurred in clinical cases but also materialize the concept of pharmaceutical care for patients＇ safe and effective use of drugs.
