人工股骨头置换治疗老年股骨粗隆间粉碎性骨折的疗效分析

Effect of Artificial Femoral Head Replacement on Senile Comminuted Femoral Intertrochanteric Fracture

目的评价人工股骨头置换治疗高龄老年人粗隆间骨折的疗效。方法对24例高龄老年人粗隆间粉碎性骨折采用人工股骨头置换治疗。骨折类型Evans分型：Ⅱ型4例，ⅢA型5例，ⅢB型7例，Ⅳ型8例。结果人工股骨头置换治疗老年股骨粗隆间粉碎性骨折手术时间短，出血量少，髋关节功能恢复好。24例随访时间6～24个月。无1例死亡，髋关节功能良好。以髋关节活动度、疼痛程度及生活自理情况为观察指标，优良率为91．7％。结论人工股骨头置换治疗高龄老年人粗隆间骨折具有创伤小，髋关节功能恢复快，显著降低骨折并发症，疗效优良。

Objective To evaluate the effect of artificial femoral head replacement on senile comminuted femoral intertrochanteric fracture. Methods 24 cases of senile comminuted femoral intertrochanteric fracture were treated by artificial femoral head replacement. According to Evans type, fracture type were type Ⅱ （four cases）, Ⅲ A （five cases）, type Ⅲ B （seven cases）, typelV （eight cases）. Results Artificial femoral head replacement for senile comminuted femoral intearochanteric fracture had the advantages of short operative time, less bleeding and well functional restgration. 24 cases were followed up in 6 to 24 months, no one died, function of coax was excellent or good in 91.7% of patients. Conclusions Artificial femoral head replacement for senile comminuted femoral intertrochanteric fracture is effective, and it has small insection and less complications.
