摘要 目的评价左室肥厚对左室收缩运动协调性和功能的影响。方法正常对照组18例(组1)、高血压性心脏病患者18例(组2)和肥厚型心肌病患者16例(组3)。应用组织多普勒成像技术测量左室收缩期电机械运动的时间离散度(DTQ-S)、室间隔和左室侧壁6个位点的心肌收缩期峰值速率(Vs)。结果与组1相比,组3DTQ-S延长(P<0.001),各位点Vs值降低(P<0.05);组2上述数值无明显变化。结论肥厚型心肌病存在左室运动收缩协调障碍和收缩功能下降。 Objective To assess the effects of ventricular hypertrophy on left ventricular systolic synchrony and systolic function by Tissue Doppler Imaging (TDI) technique. Methods 18 patients with hypertensive heart disease (HHD) and 16 patients with hypertrophic cardiomyopathy (HCM) and 18 age-matched healthy subjects as control group were performed with pulsed TDI to record the ventricular septum and left ventricular lateral, anterior and inferior walls motion pattern in the left side atrial-ventricular annulus. The time dispersion of electromechanical motion in left ventricular systole, systolic velocity(Vs) at six sites in ventricular septum and left ventricular lateral wall were measured. Results The time dispersion of electrnmechanical motion in left ventricular systole in three groups were respectively (15.6±3.5)ms (control group) vs (24.2±2.7)ms(HHD) vs (28.2±15.2) ms (HCM), it significant increased in HCM group compared with control group (P〈0.001), Vs in left ventricular segments of HCM group were lower than that of control group (P〈0.05), and there was no significant difference between HHD and control group. Conclusion Impairment of systolic synchrony of left ventricular motion and systolic function exists in HCM group.
