单边外支架治疗胫骨骨折40例临床观察

Clinical observation of single-side external fixation supporter for tibial fractures in 40 cases

目的探讨单边外固定支架治疗胫骨骨折的临床效果及技术要点。方法根据不同的手术方式,将78例胫骨骨折患者分为两组:单边外固定支架组40例,加压钢板内固定组38例,临床观察结合X线片比较两种方法的临床疗效。结果单边外固定支架组骨折愈合的优良率(87.5%)略低于加压钢板组(89.5%),但差异无统计学意义(P>0.05);两组并发症发生率相比差异无显著性(P>0.05)。主要并发症包括浅表感染3例,关节僵硬2例,畸形愈合2例,骨不连1例。结论单边外固定支架治疗胫骨骨折疗效可靠、创伤小,值得推广;严格掌握适应证、规范手术操作是改善疗效、减少并发症的关键。

[Objective] To explore the clinical effect and main operative techniques of single-side external fixa- tion supporter for tibial fracture. [Methods] According to the performed surgical styles, 78 patients with tibial fractures were divided into 2 groups： single-side external fixation supporter group （n =40） and dynamic compression plate internal fixation group （n =38）. Clinical curative effect of the two surgical approaches was comparatively analyzed by using clinical observation combined with X rays photographs. [Results] The satisfactory rate of union of fracture in single-side external fixation supporter group was slightly lower than that in dynamic compression plate internal fixation group, but the difference was not statistically significant （P 〉0.05）. There was no significant difference between the two groups in complication incidence （P 〉0.05）. The main complications included superficial infection in 3 cases, anchylosis in 2, malunion in 2 and non union in I. [Conduslon] Single-side external fixation supporter deserves generalization in treating tibial fractures for its reliable curative effect and slight trauma. Strictly controlled indication and standardized operative procedure are crucial to improving curative effect and decreasing complications.
