NUSS手术(微创漏斗胸矫形术)在治疗小儿漏斗胸中的应用

NUSS procedure for the correction of pectus excavatum

目的：探讨微创漏斗胸矫形术（Minimally invasive repair of pectus excavatum或NUSS手术）的近期疗效。方法：3例患儿均应用NUSS手术治疗，即在胸腔镜下将塑形矫正板由一侧胸腔经胸骨后心包裸区穿至对侧胸腔，翻转后固定于对侧胸骨。结果：3例均顺利完成手术。手术时间30～70min，平均47min，无术中并发症。术后住院7～12d，平均10d。3例患儿术后疼痛、发热均不超过3d；无气胸及皮下气肿；术后1个月复查无矫形板旋转，无固定片滑脱。结论：NUSS手术的优越性在于微创、美观，手术时间短，操作简单，近期效果满意，远期效果还需进一步观察。

Objective To investigate the short - term effects of minimally invasive repair of pectus excavatum （ NUSS procedure）. Method 3 patients were all treated with NUSS procedure, which were performed under general anesthesia with endotracheal intubation. A transverse incision was made in each lateral chest wall and a steel bar was passed under the sternum from the right thoracic cavity under thoracoscope, then the bar was turned over to raise the sternum to the normal position. The bar was fixed to bilateral costal bones by using two stabilizing bars. Results 3 cases all got the successful operation, without complications during the surgery. The operation time ranged from 30 to 70 min （average 47 min） and the post - operative hospital stay ranged from 7 to 12 days （average 10 days）. The post - operative pain and fever were less than 3 days and without pneumothorax and subcutaneous emphysema. No unilateral stabilizing bar displacement and incision infection were found in body examination 1 month after operation. Conclusion The NUSS procedure is safe and effective for the correction of pecrus excavatum in children, and with the advantages of microinvasive, beautiful outlook, less time during operation, simple handling and satisfactory short -term efficacy.
