摘要
β阻滞剂心得安治疗门脉高压的作用已较肯定,但在少数肝功能较差病人中效果欠佳.本研究用放射配基法检测了门脉高压症动物及患者的心肌细胞和血液淋巴细胞β受体数目及功能,以期探明部分病人对心得安无反应的机理.结果显示:1.胆总管结扎性肝硬化兔中,心肌细胞和血液淋巴细胞的Bmax(最大结合数)明显减少,且与肝功能状态有关;2.肝硬化门脉高压病人血液淋巴细胞的Bmax也明显减少,肝功能差者尤甚.由此得知,部分门脉高压症患者对心得安无反应的原因与其心血管系统β受体下调有关.
Propranolol(PRO), a beta blocker, could be effective in treating portal hypertension, however, few patients with portal hypertension do not respond to PRO. We have detected the beta receptor Bmax and Kd of myocardium and blood lymphocytes in portal hypertension. Results showed: 1. In rabbit models that were subjected to cholecystectomy and double ligation of the common bileduct(CBDL), Bmax of myocardium and blood lymphocytes decreased, concerning liver funtion. 2. Bmax of blood lymphocytes in patients with cirrhosis and portal hypertension reduced, especially in those with worse liver function. It suggested that there exist beta receptor down regulation of cardiovascular system in portal hyportension. This may explain why a part of patients with portal hypertension have no response to PRO.
