舒张性心力衰竭患者血浆脑钠素含量与心功能的关系

The Correlation between the Blood Plasma Brain Natriuretic Peptide(BNP) Levels and Heart Function in Patients with Diastolic Heart Failure

摘要 目的:探讨舒张性心力衰竭患者血浆脑纳素(BNP)水平与心力衰竭严重程度,左室功能的关系。方法:采用免疫荧光法测定48例舒张性心力衰竭患者(心衰组)和30例心功能正常的患者(对照组)血浆BNP浓度;用心脏彩色多普勒超声诊断仪测定各组左室射血分数(LVEF)。结果:心衰组血浆BNP浓度显著高于对照组(P<0.01);血浆BNP水平随着NYHA心功能分级程度的加重而显著增高(P<0.01)。LVEF随着NYHA心功能分级程度的加重而降低,但无统计学意义(P>0.05)。不同LVEF的各亚组心力衰竭患者血浆BNP水平间差异无统计学意义(P>0.05),但与LVEF呈负相关(r=-0.33,P<0.05)。结论:舒张性心力衰竭患者血浆BNP水平随着心力衰竭严重程度的加重而升高,对诊断舒张性心力衰竭具有重要的作用。 Objective: To investigate the correlation between the blood plasma brain natriuretic peptide(BNP) levels and the severity of heart failure(HF) as well as left ventricular function in patients with diastolic heart failure. Methods:Forty-eight patients with diastolic heart failure and 30 control subjects were investigated by measuring the blood plasma BNP levels by immumofluorescence method and the left ventricular ejection fraction(LVEF) by color Doppler echocardiography. Results: The blood plasma BNP levels in patients with diastolic heart failure were markedly higher than those of control subjects (P〈0.01), and the increase of the blood plasma BNP levels was significantly correlated with the severity of NYHA functional class(P〈0. 01). The decrease of LVEF was correlated with the severity of NYHA functional class, which was not statistically significant(P〉0.05). There was not statistically significant among the blood plasma BNP levels of each subset patients with HF of different LVEF (P〉0.05), but the BNP levels were negatively correlated with the LVEF(r=-0. 33 ,P〈0. 05). Conclusion:The increase of the blood plasma BNP levels in patients with diastolic heart failure is closely correlated with the se- verity of HF, which is important for the diagnosis of diastolic heart failure.
