锁定微创内固定技术治疗肱骨近端骨折临床研究

Clinical study of minimal invasive internal fixation with locking technique for treating proximal humeral fractures

目的探讨肱骨近端锁定接骨板微创内固定治疗肱骨近端骨折的临床疗效。方法自2003年6月～2006年3月,应用肱骨近端锁定接骨板微创内固定治疗35例肱骨近端骨折患者,其中男性13例,女性22例;年龄31～76岁。平均60．7岁。根据Neer分类,二部分骨折16例,三部分骨折12例,四部分骨折7例。结果随访时间为7～20个月,平均11．3个月。所有患者均达到骨性愈合,平均愈合时间为7．8周。根据Constant评分,优良率为88．6%。结论肱骨近端锁定接骨板微创内固定方法具有固定稳定、操作简单和血运破坏少等优点,适用于治疗肱骨近端骨折。

Objective To discuss the clinical results of minimal invasive internal fixation with locking proximal humeral plate （LPHP） in treating proximal humeral fractures. Methods From June 2003 to March 2006, we treated 35 patients of proximal humeral fracture through minimal invasive internal fixation with LPHP. They were 13 men and 22 women, and they were from 31 to 76 years old, with the mean age of 60.7. According to Neer classification, there were 16 cases of two-part fractures, 12 cases of three-part fractures and 7 cases of fou〉part fractures. Results The follow-up period was from 7 months to 20 months, with the mean period of 11.3 months. All patients had achieved bone union, with the mean healing period of 7.8 weeks. According to the Constant scoring, the rate of excellent and good cases was 88. 6%. Conclusions Minimal invasive internal fixation with LPHP has advantages of stable fixation, simple manipulation and less interference of blood supply, and so on. It is suitable for the treatment of proximal humeral fractures.
