彩色多普勒超声对糖尿病肾病肾脏血流动力学的观察分析

Analysis of the Renal Artery'Hemodynamics revealed by Color Doppler Ultrasound in Patients with Diabetes Nephropathy

目的:探讨糖尿病肾病肾内动脉血流变化及彩色多普勒超声的应用价值。方法:应用彩色多普勒血流显像对11例Ⅰ型糖尿病患者、37例Ⅱ型糖尿病患者及21例年龄匹配的正常人的肾主动脉(MRA)、段动脉(SRA)、叶间动脉(IRA)进行检查,并对其血流的收缩期最大血流速度(The systolic velocity:Vsmax)、舒张期最低血流速度(The diastolic velocity:Vdmin)和阻力指数(resistance index:RI)进行对比分析。结果:与对照组比较,Ⅱ组中IRA的Vdmin明显减低(P<0.05),RI增高(P<0.05)。Ⅲ组中,MRA、SRA的Vdmin显著减低(P<0.05),RI明显增高(P<0.01),IRA的Vsmax、Vdmin均明显减低(P<0.05),RI显著增高(P<0.01)。结论:RI增高和彩色多普勒声像图的改变对早期诊断糖尿病肾病及判断肾功能不全有一定价值。

Objective： To explore the changes of the various renal artery＇s hemodynamics in patients with diabetes nephropathy and the diagnostic value of the color Doppler ultrasonography, Methods： 11 patients with type Ⅰ diabetes, 37 patients with type Ⅱ diabetes and 21 healthy subjects were examined by color Doppler ultrasonography. The systolic velocity（Vsmax）, diastolic velocity（Vdmin） and resistance index（RI）of master renal arteries（MRA）, segmental renal arteries（SRA） and interlobular renal arteries（IRA） were evaluated. Results：Compared with control group, in groug Ⅱ patients, the Vdmin of IRA was markedly decreased （P〈0. 05） and the RI of IRA was significantly increased （P〈0. 05）. In group Ⅲ patients, the Vdmin of MRA and SRA was greatly lower than that of oontrol group and the RI of them was significantly higher than that of control group （P〈0. 05）, the Vsmax and Vdmin of IRA were significantly lower than that of control group and the RI of it was significantly higher than that of control group （P〈0. 05）. Conclusions： Examination of the renal artery in patients with early diabetes nephropathy and renal failure by Color Doppler ultrasound may be an important diagnostic method.
