口腔颌面肿瘤的研究及诊治10年进展

Progress and achievements in diagnosis,treatment and research of oral and maxillofacial tumors:10-year review

本文就１０年来的成就进行了回顾。在基础研究方面，我国迄今已建立有口腔鳞癌和涎腺癌细胞系６株；在此基础上，还先后获得口腔鳞癌单克隆抗体及筛选出肺高转移性囊性腺样癌细胞株。研究工作已从细胞水平向分子水平发展，各种细胞因子、癌基因、抗癌基因以及基因转导等方面的研究已在各有关实验室有计划地进行。在临床工作中，各种先进的诊断方法，诸如８超、ＣＴ、ＭＲＩ、ＤＳＡ、ＳＰＥＣＴ以及超选择性颈动脉插管介入放射等等均已被用于临床的综合诊治中。口腔癌和涎腺癌以手术为主综合治疗的５年生存率在６５％及８８％左右。为了保证病员的生存质量，我国已成为术后缺损立即整复累积病例最多的国家，显微外科技术已被普遍应用。其它疗法诸如术前辅助性化疗、热疗、热化疗、免疫治疗、冷冻治疗等新技术新疗法也有不少临床研究和取得成功的报道。近来用逆转病毒导入ＴＮＦ－α于肿瘤引流淋巴结淋巴细胞（ＤＮＬ）的基因治疗也已开始被用于临床。

Abstract Six cell lines of oral squamous cell carcinoma andsalivary carcinoma have been estab-lished,a monoclone antibody from oral squamous cell carcinoma obtained and one cell strain of adenoid cystic carcinoma which characteristic of high metastatic effect to lung was sieved further in China. The research emphasis is going from cell level to molecule one in which studies on cytokine,oncogene,antioncogene, gene transmitter and so on are being performed on schedule.Clinically,modern diagnostic means including B ultrasonic scan,CT scan,MRI,DSA,SPECT and super-selective angiography , etc. are now popularized.The 5-year survival rates of patients with oral cancer and salivary gland one are 6 5%and 88%respectively after surgery in combination with other adjuvant therapeutics. Now China has been a country which own the most accumulated cases with immediate reconstruction after ablative surgery,and microsurgical technique is quite popular for the improvement of patient’s life quality throughout the country. A lot of clinical sttidies on some other therapeutics,such as preoper ative adjuvant chemotherapy, thermotherapy,thermochemotherapy,immunotherapy,cryotherapy have been reported with satisfactory results. Gene-therapy of draining node lym-phocyte( DNL)withTNF-α transductedbyretrovirus begins to be adopted c linica lly.
