拇指多指畸形的分型及手术治疗

Classifications and surgical treatments for polydactyly of the thumb

探讨拇指多指畸形的临床分型及手术治疗。方法：通过主干指的确定，选择赘生指切除，保留副指组织修复主干指，重建拇短展肌止点，酌情截骨矫形等综合处理，重建主指力线和骨轴线，改善主干指外形和功能。结果：１９８５年至今，共治疗７６例８４指，年龄３个月至３４岁。术后随访４个月至９年，主干指外形改善好，能和小指对拇，拇外展充分。

<Abatract>Objective: To consider the classification and surgical treatment for polydactyly of the thumb. Methods: Plastic surgery was performed for correcting the adduction. lateral deviation and the other deformity of the thumb. The chief thumb was maintained and the tissues of the second-thumb was taken for repair. Results: Since 1985, 76 cases (84 thumbs) had been followed up from 4 months to 9 years after operation. The configuration and the function of the thumbs were satisfactory. Conclusions: New classification was suggested as follows: floating thumb type, joint type, bone type and bone-joint type. Thus could include the whole rang of polydactyly. The surgical treatment was chosen according to the classification. The operative age was within 4-7 months.
