中低位直肠肿瘤局部切除术的疗效观察

Observation on Therapeutic Effect of the Local Resection of Low and Middle Rectal Carcinoma

为探讨Mason手术在中低位直肠肿瘤局部切除术中的应用价值，对采用Mason手术作局部切除的42例中低位直肠肿瘤进行回顾性分析。结果显示，42例中直肠腺瘤10例，直肠癌30例，直肠类癌2例。恶性肿瘤中Tis-T1期21例，T2～T3期11例，病理证实切缘无肿瘤残留。术后随访其中1例因肝转移死亡，1例因复发行Miles手术。结果表明，Mason手术可作为直肠中低位良性肿瘤以及Tis-T1期中低位直肠癌的首选术式，对进展期直肠癌选择行Mason手术后应辅以放疗。

Local resection of low and middle rectal carcinoma in 42 cases was performed with Mason procedure and the results were retrospectively analized. Of the 42 cases, 10 cases had rectal adenoma,30 had rectal carcinoma,and 2 had rectal carcinoid, Of the 32 cases with malignant carcinoma,21 cases were at Tis-Tl stages and 11 cases were at T2-T3 stages. Pathological examination showed no carcinoma residual at the incisal margins. In follow up,one case was found dead of hepatic metastasis,and one case underwent Miles operation due to relapse. It is suggested that Mason procedure may become the procedure of first choice in surgical treatment of low ＆ middle rectal benign tumours and of low ＆ middle rectal malignant tumours at Tis-T1 stages;and that postoperative radiotherapy should be added after Mason procedure was applied in the surgical treatment of rectal carcinoma at the advanced stages.
