摘要
目的探讨儿童非霍奇金淋巴瘤(non-Hodgkin lymphoma,NHL)病理组织中细胞外基质金属蛋白酶诱导因子(extracellular matrix metalloproteinase inducer,EMMPRIN)和基质金属蛋白酶9(matrix metalloproteinase-9,MMP-9)的表达,以及两者表达间的相关性。方法免疫组织化学SP染色方法检测47例NHL病理切片中的EMMPRIN和MMP-9的表达,采用Spearman等级相关分析方法分析EMMPRIN表达与MMP-9的相关性,同时分析其与临床分期及预后的关系。结果①EMMPRIN在儿童NHL的表达阳性率为81%(38/47),其中T细胞型NHL的强阳性率为76%(19/25),B细胞型NHL的强阳性率为36%(8/22),两者强阳性表达率差异有极显著性意义(P<0.01)。②MMP-9在儿童NHL中的表达阳性率为79%(37/47),其中T细胞型NHL的强阳性率为72%(18/25),B细胞型NHL的强阳性率为23%(5/22),两者强阳性表达率差异有极显著性意义(P<0.01)。③EMMPRIN的表达与MMP-9有显著正相关性(rs=0.913,P<0.05)。④Ⅲ期+Ⅳ期组以及存活时间<5年组EMMPRIN、MMP-9的强阳性表达率均分别高于Ⅰ期+Ⅱ期组和存活时间≥5年组。结论儿童NHL细胞高表达EMMPRIN和MMP-9,且T细胞型NHL的表达明显高于B细胞型,两者表达强度与临床分期及预后有关,可能作为NHL预后判断指标之一。
Objective To investigate the expression of extracellular matrix metalloproteinase inducer (EMMPRIN) and matrix metalloproteinase 9 (MMP-9) in the children with non-Hodgkin lymphoma (NHL) and to test the correlation between EMMPRIN and MMP-9. Methods Streptavidin/peroxidase (SP) immunostainings were performed to detect the presences of EMMPRIN and MMP-9 in pathological sections from 47 children with NHL and the associations of the expression with clinical features. Spearman rank correlation was executed to analyze the relationship between the expression of EMMPRIN and MMP- 9. Results The positive expression rate of EMMPRIN was 81% (38/47). The rate of strong expression was 76% (19/25) and 36% (8/22) in T cell NHL and B cell cases, respectively with the difference between them being statistically significant (P〈 0.01). The expression rate of MMP-9 was 79% (37/47), strong expression was found in 72% (18/25) of T cell cases, and that in 23% (5/22) of B cell cases with the difference being significant between them (P〈0.01). MMP-9 was directly correlated with the expression of EMMPRIN (rs =0. 913, P〈0. 05). The strong expression rate of EMMPRIN and MMP-9 was high in cases of clinical stage Ⅲ and Ⅳ or survival time less than 5 years, whereas low in stage Ⅰ and Ⅱ or survival time more than 5 years. Conclusion Most of cases of NHL expressed EMMPRIN or MMP-9, and the expression of EMMPRIN or MMP-9 in T cell cases was much stronger than that in B cell ones. Over-expression of EMMPRIN or MMP-9 may be correlated with a poor prognosis.
