慢性湿疹589例斑贴试验结果分析

Evaluation of the results and clinical significance of patch testing in 589 out-patients with chronic eczema

为了解我国北方地区慢性湿疹患者斑贴试验结果及临床意义,回顾性分析了本地区近20年来积累的589例慢性湿疹患者斑贴试验结果,分析接触性变应原在不同性别、年龄、季节及年代的分布特征。结果斑贴试验阳性者269例,占45.7%。阳性率居前9位的接触性变应原分别为:20%松香5.9%(35例)、1%甲醛5.3%(31例)、1%次乙基二胺4.2%(25例)、30%羊毛醇4.1%(24例)、5%苯唑卡因3.6%(21例)、8%芳香混合物3.2%(19例)、5%硫酸镍2.7%(16例)、0.5%对苯二胺2.7%(16例)、3%卡巴混合物2.5%(15例)。接触变应原分布与性别、年龄、季节、发病年代等因素无关。说明慢性湿疹病人接触性变应原斑贴阳性率较高,外源性接触性变应原可能是慢性湿疹的重要致病因素。

To evaluate the results and clinical significance of patch testing in chronic eczema, 589 out-patients with chronic eczema were patch tested during the last 20 years. It was found that 45.7% （269/589） patients with chronic eczema had positive reactions to the 20 allergens tested. Among them, positive rates were 20% colophony 5.9% （35/589）, 1%fonnaldehyde 5.3% （31/ 589）, 1%ethenyldiamine4.2% （25/589）, 30%lanonol 4.1% （24/589）, 5% benzecain 3.6% （21/589）, 8% fragrance mix 3.2% （19/589） , 5%nickel sulfate 2.7% （16/589）, 0.5%paraphenylenediamine 2.7% （16/589） and 3% earba mix 2.5% （15/589）. There was no correlation between distribution of contact allergens and factors including gender, age, season or onset era. It indicated that the positive rate of contact allergens is high. Contact allergens may take part in the pathogensis of chronic eczema.
