经皮后凸畸形椎体成形术治疗脊椎溶骨性转移瘤

摘要 目的探讨经皮后凸畸形椎体成形术结合放疗治疗脊椎溶骨性转移的疗效。方法对6例脊椎溶骨性转移瘤患者共8个椎体在C型臂-X线机监视下经皮椎体穿刺,椎进~特殊高压球囊,扩张形成一小空腔,注入聚醛丙烯酯填充;手术2周后,应用局部放疗外照射。结果6例(8个椎体)一次性穿刺、灌注均成功,CT显示灌注剂充填满意,无椎管内或椎间孔渗漏;术后随访3~22个月,1例死亡,余5例腰背部活动改善,疼痛症状明显缓解。结论经皮后凸畸形椎体成形术治疗脊椎溶骨性转移安全可行,方法简单,术后立即缓解疼痛,结合放疗,效果满意。 Objective To explore the therapeutic effects of treating spinal ostenlytic metastases tumor with pereutanenus vertebroplasty (PV) combined with radiotherapy . Methods Pereutanenus puncture was carried out on 8 vertebral bodies of 6 patients under the guide of C - X - ray , a specific high tensioned balloon with was introduced and extended to form a space which was then filled with acrylic dentyre; an locally external radiotherapy was done 2 weeks postoperatively . Results The pereutanenus vertebroplasty was successfully done on all the 6 patients , the filling of acrylic dentyre was satisfactory according to CT scan . There was no leakage either intra vertebral canal or at intervertebral foramina, a follow up was performed 3 - 22 months, 1 patient died , the 5 others improved in back activities and had a significant pain relief. Conclusions Treating spinal estenlytic metastases tumor with pereutanenus vertebroplasty(PV) combined is safe and simple for clinical application .The pain can be released right after operation . The therapeutic effect is satisfactory when combined with radiotherapy .
