摘要 目的:研究神经生长因子(NGF)高亲和受体TrkA与低亲和受体p75在胰腺癌中的表达意义.方法:应用实时定量PCR法检测56例胰腺癌组织中TrkA及p75的表达并进行临床病理及预后分析.结果:所有组织标本中均可检测出TrkA及p75的表达呈负相关.p75与TrkA的表达水平与患者的腹背部疼痛和神经浸润程度有关(P均<0.05).Cox单变量分析揭示,高p75表达患者较高TrkA患者的总生存时间延长(P<0.05),Cox多变量分析揭示,p75及TrkA均为影响预后的独立因素.结论:胰腺癌中TrkA的表达促进肿瘤的恶性进展,而p75则相反,NGF刺激增长或抑制胰腺癌的双重作用可能与TrkA及p75的表达比率相关. AIM: To investigate the prognostic values of 2 nerve growth factor receptors ( NGFR), high-affinity receptor TrkA and low-affinity receptor p75, in pancreatic cancer. METHODS: We detected the expressions of TrkA and p75 mRNA in 56 cases of primary pancreatic cancer by using real-time quantitative RTPCR. Their clinical manifestations, pathological characteristics and prognosis were analyzed comparatively. RESULTS: TrkA and p75 were both found in all 56 cases of pancreatic cancer. TrkA and p75 levels were correlated with abdominal or back pain and perineural invasion ( P 〈 0. 05 ). Cox univariate analysis revealed that the cases with higher expression of p75 survived the ones with higher expression of TrkA ( P 〈 0. 05 ) , and Cox multivariate analysis indicated that TrkA and p75 were both the independent factors influencing the prognosis. CONCLUSION: The expression of TrkA in pancreatic cancer is a marker of tumor aggressiveness; conversely, the elevated p75 expression is associated with a favorable prognosis. NGF exerts stimulatory or inhibitory effects on pancreatic cancers, which depends on the expression levels and ratio of TrkA to p75NGFR.
