疑难病例析评第121例反复肢体皮下血肿—未明原因的高γ球蛋白血症—骨痛—凝血因子Ⅷ部分缺乏

病例摘要
患者，女，57岁，因反复发作肢体皮下血肿14年于2004年5月11日入中南大学湘雅二医院。患者于1990年7月无明显诱因出现左下肢皮下血肿，血肿逐渐增大，约拳头大小，肿胀，局部波动感明显，伴疼痛，数天后自行消退。此后多次无诱因于全身多个部位出现过大小不一的皮下淤斑或血肿，在外院经对症处理后消退。2004年5月患者因被小狗脚爪轻度抓伤后出现右肘关节处皮下血肿，并渐行性增大成水囊样，局部疼痛剧烈，活动受限而急诊入院。既往有“二尖瓣脱垂”病史20年，15年前体检时发现血清球蛋白增高，未重视。1987年接受双侧扁桃体摘除术，当时止血良好，无过度出血倾向。30年前顺产1女，无过度出血。月经规律，经量正常。否认异烟肼等药物摄入史，否认青霉素类药物的长期注射史。父母非近亲婚配。家族中无出血病史者。
