肺结核并发急性肾功能衰竭37例临床分析

Clinical analysis of 37 pulmonary tuberculosis patients with acute renal failure

目的分析肺结核并发急性肾功能衰竭的原因及其特点,探讨预防、治疗措施。方法对37例肺结核并发急性肾功能衰竭的病例进行回顾性分析。结果本组37例肺结核并发急性肾功能衰竭中治愈好转18例,放弃治疗自动出院3例,死亡16例。行血液透析治疗的9例患者预后良好。结论肺结核治疗后并发急性肾功能衰竭的发生,除与发病年龄、原有基础病有关外,还与抗痨药的使用关系密切,如发现急性肾功能损害应即停用肾毒性的抗痨药,积极对症治疗,尽早行透析治疗,可提高治愈率。

Objective To analyze the causes and characteristics of pulmonary tuberculosis accompanied with acute renal failure and to probe prevention and therapy measures of the disease. Methods The data of 37 patients with pulmonary tuberculosis accompanied with acute renal failure were retrospectively reviewed. Results Among the data, 18 cases were cured, 3 cases were abandoned or left hospital voluntarily, 16 cases died, 9 cases recovered after blood dialysis. Conclusion Beside the age and basic diseases of patients, the usage of anti-tuberculosis drugs is closely related to the onset of pulmonary tuberculosis accompanied with acute renal failure. If the acute renal failure is found, it is necessary to stop using the nephrotoxic anti-tuberculosis drugs as soon as possible. It is important to active treatment for the complicated disease and to give blood dialysis for the patients as early as possible.
