阴道镜对宫颈疾病的诊断价值

Clinic application for cervical diseases diagnosing through colposcopy

目的评价阴道镜对宫颈病变尤其是宫颈上皮内瘤样病变（CIN）的诊断价值。方法2005年5月至2006年10月该站通过阴道镜对宫颈病变进行检查，以组织学为金标准进行结果分析。结果阴道镜检查538例，133例宫颈取活组织检查，病理结果：CIN110例，其中CINⅠ48例（36．1％），CINⅡ11例（8．3％），CINⅢ51例（38．4％），人乳头状病毒（HPV）感染8例（6．0％），宫颈炎性反应15例（11．3％）。阴道镜诊断CIN的检出率为20．4％．敏感性97．3％，特异性65．2％。阳性预测值82．7％，阴性预测值92．0％。结论采用阴道镜能及早发现宫颈癌前病变。

Objective To evaluate the benefits of the colposcopy for the diagnosis in cervix diseases especially in CIN. Methods During the time between May in 2005 and October in 2006 ,in the service station for family planning colposcopy was used to diagnose the cervical diseases and the histology was used as the gold standard in analyzing the results. Results In colposcopy test of 538 cases, 133 cases were tested under the cervical biopsy. Pathological results showed：a total of 110 cases of CIN uncovered,in which there were 48 cases of CIN Ⅰ （36.1% ）, 11 cases of CIN Ⅱ （8.3 % ）, 51 cases of CIN Ⅲ （38.4 % ）, 8 cases of HPV infections（6.0 % ）, and 15 cases of chronic cervical inflammation（ 11.3 % ）. The colposcopy for diagnosing CIN reported a effectivity of 20.4 percent, a sensitivity of 97.3 percent and a specificity of 65.2 percent. Positive prediction data was 82.7 % and negative prediction data was 92.0 %. Conclusion The colposcopy can detect and reveal pre-cancer diseases and anomalies earlier.
