胸椎间盘突出症手术治疗的并发症分析

Analysis of the Complications Second to the Treatment of Thoracic Disc Herniation Using Transpedicle Entrances

目的探讨经关节突入路治疗胸椎间盘突出症手术并发症的原因及治疗方法。方法回顾性分析我院2000年11月至2005年11月间收治的52例胸椎间盘突出症病例，采用关节突关节切除的后外侧入路手术。其中男32例，女20例。男女之比为1：0．625。年龄31．5～69．5岁．平均45．5岁。术后随访时间为4～54个月，平均34个月。观察患者发生并发症的情况。用SPSS13．0进行统计学处理。结果52例患者中出现并发症者12例。其中术后切口肿胀3例，神经系统功能障碍加重1例，脊髓反应性水肿1例，下肢深静脉血栓形成2例，伤口感染1例，内固定失败2例，脊前动脉综合征1例。脑脊液漏1例。所有患者出现并发症后均得到及时的处理。结论经关节突关节入路治疗胸椎间盘突出症效果良好．术者熟练的操作、规范的术前准备和术后处理在防治并发症上有重要作用。

Objective To analysis the complications of the transpedical entrance to treat thoracic disc herniation, analysis the reason and the methods preventing it. Methods Retrospective analysis the patients treated in our hospital from Nov. 2000 to Nov. 2005, who were treated with the transpedicle entrance in our hospital. There are 52 patients,32 males,20 females,the age of this group is among 31.5-69.5 years old,and 45.5 years for average. We got about 4-54 months following-up,and 34 months for average. Collecting all the complications occurred in the followup time. Using SPSS 13.0 to do the statistics works. Results 12 of all the 52 patients had found the complications occurred. 3 patients for incision swelling; 1 patient neurological dysfunction aggravated;1 patient was found spinal cord reactive edemas2 patients thrombosis in 1 patient, infection of the wound 2 patients inter-fixation system failed in 1 patient anterior spinal artery syndromes1 patient Leakage of CSF was observed. All the patients were successfully treated. Conclusion Transpedicle entrance is a good method in the treatment of the thoracic disc herniation. And only the operator is familiar with the standard operation method,while the pre-operation preparation and the post-operation care was performed carefully,the operation will be safe and effective.
