结节性甲状腺肿的内镜治疗(附80例报告)

Endoscopic therapy of nodular goiter (A report of 80 cases)

目的探讨内镜甲状腺切除术治疗甲状腺良性疾病的可行性及效果。方法80例结节性甲状腺肿患者行经前胸壁途径的内镜甲状腺切除术,结节直径10～45mm。结果手术全部成功。手术时间120～210min,平均150min。出血量20～70mL,平均30mL。单侧甲状腺大部切除57例;一侧甲状腺大部切除、另一侧部分切除23例。术后2、3d拔除引流管。术后平均住院5.4d。无神经、血管、甲状旁腺损伤等并发症,颈部无疤痕。结论内镜甲状腺切除术安全可行,有优良的美容效果,是甲状腺良性疾病的理想术式。

[Objective] To discuss the feasibility and effect of endoscopic thyroidectomy to treat benign disease of thyroid. [Methods] Endoscopic thyroidectomy via anterior chest wall approach was performed in 80 patients of nodular goiter with diameter between 10 mm and 45 mm. [Results] All operations were successfully carried out. The mean operative time was 150 min （range, 120-210 min）, and the mean blood loss was 30 mL （range, 20-70 mL）. Subtotal thyroidectomy of single lobe was 57 cases; subtotal thyroidectomy of single lobe and partial thyroidectomy of another lobe was 23 cases. Remove drainage tube between 2 days or 3 days after operation. Mean postoperative hospital stay was 5.4 days. There were no complications such as damage to nerve, blood vessel and parathyroid glands, and no scars in neck. [Conclusion] Endoscopic thyroidectomy is a safe and effective method, and the obvious cosmetic effect of this method is guaranteed. This method is the ideal operative style of benign disease of thyroid.
