卡孕栓预防高危妊娠剖宫产产后出血临床观察

Clinical Observation of Carboprost Suppository in Reducing Postpartum Hemorrhage After Cesarean in High Risk Pregnancy

目的观察卡孕栓预防高危妊娠剖宫产产后出血的效果。方法将153例高危妊娠剖宫产产妇分为3组，分别为舌下含服卡孕栓组、舌下含服米索前列醇组和静脉滴注缩宫素组，均于胎儿娩出后立即用药，比较剖宫产术中、术后2h及术后2～24h出血量。结果卡孕栓组术中及术后2h出血量均明显少于和缩宫素组，产后出血发生明显减少（P〈0．05），与米索前列醇组比较，术中出血量明显减少（P〈0．05），术后2h出血量无显著性差异（P〉0．05）。用药前后产妇血压无显著变化，副反应少，术后肛门恢复排气时间缩短。结论卡孕栓舌下含服能明显减少高危妊娠剖官产产后出血量，且应用简单、安全可靠，值得推广。

Objective To investigate the effect of carboprost suppository in reducing the postpartum hemorrhage after cesarean birth among high risk pregnancy. Methods Altogether 153 high risk pregnancy women accepted cesarean delivery were randemly divided into three groups： the group of taking carboprost suppository sublingually, the group of taking misoprostol subiinguaiiy and the group of injecting oxytocin intravenously. The medicine was taken immediately after child delivery and the amount of bleeding during operation, 2 hours after operation and 2-24 hours after operation was compared respectively. Results During the operation and 2 hours after operation the hemorrhage quantity of the carboprost suppository group was reduced significantly and the incidence of postpartum hemorrhage was significantly less than the group injected oxytocin （P〈 0.05）. The hemorrhage quantity of the carboprost suppository group during operation was reduced significantly than that of the misoprostol group （P〈 0.05）, while there was no significant difference between the two groups 2 hours after the operation （P 〉 0.05）. The blood pressure fluctuation was slight, the side effect was few and anus vent recovered quickly after taking carboprost suppository sublingually. Conclusion Taking carboprost suppository sublingually can significantly reduce hemorrhage after cesarean delivery among high risk pregnancy, and with the advantages of convenient administration, safety and reliability.
