结核性腹膜炎的超声显像诊断分析

The Analysis of Ultrasound Diagnosis on Tuberculous Peritonitis

本文报道我院应用超声检查经手术病理和腹腔镜证实的73例结核性腹膜炎的结果。探讨声像图特点及诊断价值。结果:超声诊断符合61例,确诊率为83.7%。声像图分三型:(1)腹水型33例。(2)光团型15例。(3)混合型25例。讨论中指出结核性腹膜炎在病理上分为渗出型、粘连型、干酪型。三种不同病理变化形成了三种不同的声像表现。腹水型的病理基础是渗出型,有其独特的声像特点.应与肝硬化腹水鉴别。花团型的病理基础为粘连型,此型最易误诊。混合型的特点是液实并存,其病理基础是干酪型。本病术前误诊率为15~47%,本组B超确诊率为83.7%,是诊断本病的安全可靠简便的方法。

73 cases with tuberculous peritonitis were diagnosed by OS and postoperation pathology. The results showed accuracy diagnosis rate was 83. 7% (61/73). The sonogram divided three types; Dascites 33 cases, 2)photoba]l 15 cases. 3)mixing 25 cases. The tuberculous peritonotis divided three types-, exudative . adhesive and caseification pathology, formed three types sonogram feature. The ascites type pathological basis was exudative and it's unique sign was distinguish from the ascites of cirrhosis. The photoball type was resulted from pathological adhesive and easily misdiagnosis. The mixing type was formed by pathological caseification. This disease preoperation misdiagnosis rate was 15 -47% , and US diagnosis accuracy rate 83. 7%. The US diagnosis is a safe, reliabe and handy method.
