颈动脉连续波多普勒超声评估颅内侧支循环 被引量:5

摘要 目的探讨颈动脉连续波多普勒超声评估颅内侧支循环。方法对80例无脑供血动脉狭窄性病变患者,在行颈总动脉压迫试验时采用4 MHz连续波探头检测对侧颈内动脉起始段和同侧椎动脉寰枢段血流速度变化。并对连续波多普勒超声与经颅多普勒超声检测的侧支循环方式结果进行比较。结果颈动脉连续波多普勒超声检出前交通动脉侧支开放76例(95%),经颅多普勒超声检出前交通动脉侧支开放76例(95%)。在受检的160个后交通动脉侧支中,颈动脉连续波多普勒超声检出后交通动脉侧支开放140个(88%),经颅多普勒超声检出132个(83%)。结论颅外动脉连续波多普勒超声结合颈总动脉压迫试验检测颅内侧支循环既安全又方便,其准确率与经颅多普勒超声一致。 Objective To assess the intracranial collateral circulation with cervical continuous-wave Doppler. Methods In 80 patients without stenosis disease of cerebral and carotid artery, the blood flow velocity changes in the original parts of contralateral internal carotid artery and atlantoaxial parts of ipsilateral vertebral artery were measured with a 4 MHz continuous-wave probe during common carotid artery compression, We compared the collateral circulation patterns identified by both transcranial Doppler and continuous-wave Doppler. Results Cervical continuous-wave Doppler identified the anterior communicating artery patent in 76 patients (95 %). Transcranial Doppler identified the anterior communicating artery patent in 76 patients(95% ). In 160 posterior communicating artery, cervical continuous-wave Doppler identified 140(88 % ) posterior communicating artery patent, transcranial Doppler identified 132(83 % ). Conclusions This study concludes that continuous-wave Doppler of the extracranial arteries combined with common carotid artery compression is a safe and easy way to detect intracranial collaterals, with an accuracy equivalent to transcranial Doppler.
