摘要
目的探讨口腔鳞状细胞癌凋亡与肿瘤血管生成的关系。方法60例口腔鳞状细胞癌组织为恶性组,10例口腔良性肿瘤为良性组,10例正常人牙龈组织作为正常组,观察口腔鳞状细胞癌的凋亡指数(AI)与癌灶内微血管密度(IMVD)的关系。肿瘤癌灶内IMVD的检测采用CD34抗原SP免疫组织化学染色;凋亡检测采用DNA原位末端标记检测法(TUNEL法)。结果3组IMVD差别有统计学意义;TNM分期与IMVD有关;IM-VD与口腔鳞状细胞癌的颈淋巴结转移有密切的关系。恶性组AI低于正常组及良性组(P<0.01),而正常组与良性组之间差别则无统计学意义(P>0.05)。TNM分期与AI有关;颈淋巴结转移与AI无明显相关。结论口腔鳞状细胞癌的IMVD不仅与肿瘤的TNM分期、分化程度、颈淋巴结转移有关,而且与其AI也有密切的关系。
Objective To clarify the relation of intratumoral microvessel density(IMVD) and apoptosis in oral squamous cell carcinoma(SCC). Methods A total of 60 cases of oral SCC were enrolled in this study. Routinely formalin-fixed, paraffin-embedded tissue blocks containing the principal infiltrating tumoral mass were selected. Serial sections were cut from paraffin blocks. 10 normal epithelial cases and 10 benign tumors were as normal groups. The apoptosis index(AI) was measured by using TdT-mediated dUTP-biotin nick end labeling(TUNEL), the IMVD using an anti-CD34 monoclonal antibody. Results Apoptotic index of oral SCC was significant lower than that of the normal and benign and significant differences were observed in difference TNM groups also. The IMVD of the oral SCC was (39.45±8.98) higher than the normal and benign. Correlation of IMVD, TNM, and cervical lymphatic metastasis were found. There was a linear positive correlation between IMVD and AI(r=-0. 8006, P〈0.01). Conclusion The incidence of apoptosis in oral SCC is significantly influenced by the extent of neovascularization and suggest that tumor angiogenesis may contribute to a reduction of apoptosis in oral SCC.
