超声乳化术中晶状体后囊破裂原因与处理

目的探讨白内障超声乳化手术中晶状体后囊破裂的原因和处理。方法回顾性分析639例696眼白内障超声乳化术患者的临床资料,对术中发生后囊破裂的原因和处理进行分析总结。结果38眼出现后囊破裂(5.46%),其中撕囊时5眼,水分离时6眼,超声乳化过程中14眼,吸取皮质时13眼。术后视力≥0.3者33眼。术后16眼角膜水肿;1眼后囊破裂口较大,植入前房型人工晶状体;2眼眼压增高,用药后眼压得以控制。结论晶状体后囊破裂是超声乳化术中常见的并发症,可发生于诸多步骤,谨慎操作可降低其发生率;正确处理可获得良好的术后视力。

Objective To explore the reasons and tratment of posterior capsule rupture in phacoemulsification. Methods The clinical data in 639 cases （696 eyes） performed with phacoemulsification was retrospective analyzed, the reasons and treatment of posterior capsule rupture in phacoemulsification was investigated. Results The posterior capsule in 38 eyes were ruptured （ 5.46% ）, including 5 eyes in capsulorhexis, 6 eyes in hydrodissection, 14 eyes in the process of emulsification and 13 eyes in the cortex extraction. After the treatment, the visual acuity in 33 eyes was equal to or more than 0.3 ; The corneal edema appeared in 16 eyes,the anterior chamber intraocular lens was implanted into 1 eye, the intraoctdar pressure increased in 2 eyes and controled after treated with drug. Conclusion Posterior capsule rupture is a familiar complication in phacoemulsification, and happens in many steps. The cagy operation can lower it＇s happen rate and obtains good visual acuity.
