外伤性迟发性脑挫裂伤的CT诊断(附18例报告)

CT diagnosis of post-traumatic delayed-type contusion of brain:a report of 18 cases

目的探讨外伤性迟发性脑挫裂伤的CT特点、指征及出现时间。方法18例外伤性迟发性脑挫裂伤患者均经颅脑CT扫描,主要对迟发性挫裂伤的时间、部位、大小等进行了分析。结果迟发性脑挫裂伤的出现时间为外伤后24h之后,血肿较小,居脑外围,额叶最多。结论本研究发现迟发性脑挫裂伤最佳检查时间为外伤后48～72h。对临床症状持续加重的患者,应复查CT。

Objective To investigate the CT features, clinical signs and onset times of post - traumatic delayed - type contusion of brain. Methods Eighteen patients with post - traumatic delayed - type contusion of brain underwent CT scans of head after craniocerebral trauma. The onset time, site and size in volume of delayed - type contusion loci of brain were emphatically analyzed. Results The onset time of delayed - type contusion of brain was seen in the first 24 hours latter of craniocerebral trauma. Hematomas which were secondary to contusion of brain were less in volume and located in the periphery of brain parenchyma, while the most of which distributed in lobi frontalis. Conclusion The study demonstrated that, the optimal detection time of post - traumatic delayed - type contusion of brain is in the first 48 - 72 hours after trauma. For the patients whose clinical symptoms are continued deterioration, they should be reexamined timely by CT.
